Conference Registration (Now Open)

All registrations are to be completed online at http://lamath.org/conference2012/registration/.

  • Complete the online form (do not print this form, print the invoice page that follows after hitting the submit button). Links are provided, within the form, to view the extended sessions listings.
  • For extended session. Be sure to list all 3 choices if you are registering for the extended sessions. We will do our best to give you your first choice, but a second and third choice are needed in case that session is not available. The earlier your payment is received, the better the likelihood you will be placed in your first choice. Extended session registrations must be postmarked by October 1, 2012.
  • Once the form is completed, check the yellow box and hit the submit button below it, you will then be taken to an invoice screen where you will be provided with an invoice to print and submit with payment by mail.

For those coordinating group registrations:

  1. Have each individual complete a registration form online, submit it, print the invoice that follows, and give a copy of the invoice to you.
  2. If an individual indicated anything that the funding agency will not pay for (the luncheon, the SciPort event, Carol Myer scholarship, extended/short course session), have the individual write a check for the added amount and secure it to their registration invoice that is printed (with a paper clip not a staple).
  3. Total the balances that are covered for all of the invoice receipts and submit a check for the total that is funded, along with the supplemental checks from the individuals who added additional items to their registrations that are not funded, and the invoice receipts that each person printed to the address specified on the receipts.
  4. Remember, the earlier the payment is postmarked, the greater the discount. All extended session/short course registrations must be postmarked by October 1, 2012 and the earlier the payment is received, the more likely it is that the attendee will get their first choices of extended sessions.
